FAQ: Can we use an electronic entry/exit form?

Yes! You can use an electronic form, like a fillable pdf or Word document, as long as long as the form is:

1. Scannable

AND

2. Searchable by student name

AND

3. Printable on demand

Per the MDE Adult Basic Education office, the Tennessen Warning does not require a student signature. When you give a Tennessen warning, you are informing a student that they must give some of their personal information, and what you will do with that information. When the student provides you with their information, this indicates their consent.

Your district or organization may require a signature for other reasons (such as for photo or data release). In this case, you can investigate options such as those offered by Adobe Acrobat for obtaining virtual signatures. We recommend you work with your district or organizational leadership to find options that will meet your requirements.
